Scam Description
Hi,
They emailed me (they used Express Scripts “partnership”) about there health program. they gave me a code and I filled up an application for their service and indicated the health conditions, then they denied me based on that, saying I am qualified only for “Prevention only” and requested to file the application again.
” the access code you provided does not match the condition reported on your application and we were unable to verify your eligibility for Omada For Diabetes + Hypertension program; only the Omada Prevention Program. We regret to say we cannot accept your application at this time.”
I filed the application again, but there is no option to select “Prevention Only” so I didn’t check any conditions.
Now they reply with “After careful consideration, we’re unable to accept you into the program at this time. Based on the information you provided, you are considered low risk for the conditions addressed by this program.”
This is a phishing attack to get my personal information
